The first step toward achieving a beautiful, healthy smile is to schedule an appointment. To schedule an appointment, please complete and submit the request form below. You have two options. Our in-office visit is where our treatment coordinator and orthodontist will give you comprehensive information and details on your treatment options with a clinical examination and records. Our virtual preliminary exam option is where our treatment coordinator and orthodontist gives you an idea of your treatment options and timing. Records and an in office exam will follow the virtual exam. After requesting your appointment, our scheduling coordinator will contact you soon to confirm your appointment.
